Job DescriptionThe Intake Specialist reports directly to the
Executive Director, and is assigned to the Supportive Living
Services (SLS) Division at Spread Your Wings, LLC. As a member of
the SLS team, the Intake Specialist serves as the liaison between
Spread Your Wings, LLC and the various Regional Centers that
provide client referrals and resources. As an essential member of
the SLS team, the Intake Specialist will be expected to perform
their job duties autonomously and effectively communicate with all
other members of the Spread Your Wings, LLC team on a regular
basis. General duties will involve ongoing, consistent
communication with Case Managers and Program Managers of the
various branch offices, and occasional communication with executive
team members, as needed. As a member of the Clinical Team, the
Intake Specialist does not have any supervisor/management duties,
but will require some leadership and initiative to complete job
duties effectively, providing essential services to clients, family
members, regional center personnel, and other entities to ensure
appropriate services are delivered.Duties Include:- -Serves as the
point-person for all affiliated Regional Centers (Alta California
RC, North Bay RC, RC of East Bay, etc.) to receive referral
inquiries and relevant information-sharing in regard to intakes and
admissions.- -Responsible for completing all relevant Intake
documentation to ensure other departments receive accurate
information regarding new clients being referred to their
respective branch offices. This includes both internal and external
documentation that is essential to Intakes/Admissions.- -Will
partner with leadership team members (Case Managers, Program
Managers, etc.) to ensure that all identified needs of referred
clients are in place to ensure smooth admissions process (POS
confirmed in system, confirmation of housing, following up on any
household items that are needed, etc.)- -Actively communicating
with various Service Coordinators from regional centers to seek new
potential referrals for SLS Services. This includes phone calls,
emails, and in-person meetings, as needed. (Requires occasional
travel to various locations)- -Coordinate with the Recruiter (and
other HR personnel, if needed) to ensure appropriate staffing is in
place for new SLS clients. If staffing is not fully in place,
Intake Coordinator will negotiate a new Intake date with referring
Service Coordinators.- -On a weekly basis, will schedule and
participate in meetings with the individual branch Program Managers
and Executive Directors to evaluate the capacity for new potential
referrals/intakes for respective branch offices. (Requires
occasional travel to different office locations)- -Will participate
in public speaking/presentations to various audiences that help
generate interest from potential clients/families. This can include
small and large-scale presentations.Experience/Skills Required:-
-Minimum of at least one year of experience working with
individuals with intellectual disabilities or similar population
(mental health, behavioral health, social services, homelessness,
etc.)- -Minimum of at least one year of experience providing direct
support/interactions with social services entities (Regional
Centers, Probation Department, Department of Family & Children
Services, Health/Hospital Systems, etc.)- -Must have ability to
work independently with minimal direct supervision and perform at a
high level- -Must have familiarity and/or experience with
multi-tasking and various documentation standards- -Must be
proficient in general computer skills (emails, smart phone apcps,
internet, electronic documentation, etc.)- -Proficient writing
skills (job requires some technical writing skills associated with
medical/social services field)- -Able and available to
drive/transport to different work locations/sites, as needed- -Must
be available to work a flexible schedule to meet needs of work
assignments (occasional evenings and weekends are
expected)Preferred (but not required) Experience/Skills:-
-Familiarity with Intake procedures of Regional Center clients and
programs- -Experience/knowledge of various Regional Centers in
California, along with its individual locations and key personnel-
-Prior experience assisting others with budgeting, organization,
and other similar tasksEducational Requirements and Preferences:-
-High school diploma/GED required- -AA or above degree preferred-
-Bilingual candidates preferredSpread Your Wings is an EOE
EmployerJob Type: Part-time (20 hours/week - may increase in
future); This is NOT a remote position.Pay Range:
