Job Description:

To plan, implement, and analyze lifestyle-change program activities to members of Partnership Healthplan. Support participants, provide guidance, and lead members to develop sustainable healthy lifestyles. On behalf of Partnership Healthplan, engages members individually in groups and through multiple modalities that may include telephonic, web-based tools, and through participation in community events.Perform outreach campaigns to members supporting wellness behaviors such as well-member PCP visits, screening tests, or supporting other HEDIS measures on behalf of the organization, Assist with disseminating member incentives, when appropriate.Effectively communicate information to diverse individuals face-to-face, by phone, email, text, web-enabled tools, group presentations, or by other means. Maintain records documenting outreach activities.Leverage motivational interviewing tools to work with assigned member caseload to help members identify opportunities to improve and support healthy lifestyle change. Refer members to appropriate screening tools, identify self-management tools, educational material, and special training needs. Monitor and track member use of referred resources and provide recommendations for resources needed. Works closely with other departments to identify opportunities for member outreach events/strategies. Collaborate with appropriate departments to develop communication strategies, materials for outreach events and member handouts.Establish relationships with regional, county, and community partners where members receive treatment and services to promote exchange of information and ideas on behalf of PHC members.Gather information and provide feedback to leadership regarding the barriers members face in accessing wellness care. Provide recommendations on means to promote members knowledge and resources to health and wellness. Other duties as assigned.Education and ExperienceAssociates Degree in Health Education, Public Health, Community Health, Licensed Vocational Nurse, or related field required; Experience in Public Health Education and community outreach. Lifestyle Coach certification or Motivational Interviewing training preferred.Special Skills, Licenses and CertificationsCertification in disease specific education preferred (I.E. Diabetes Educator; Asthma Educator). Valid California driver's license and proof of current automobile insurance compliant with PHC policy are required to operate a vehicle and travel for company business. Bi-lingual preferred in Spanish, Tagalog, or Russian.Performance Based CompetenciesStrong, effective oral and written communication skill. Ability to work with ethnically and culturally diverse populations. Requires patience and ability to work with people of all levels of understanding and education. Proficient in multiple technological platforms (i.e., text, email, web-based portals, etc.)Work Environment And Physical DemandsAbility to use a computer keyboard. More than 80% of work time is spent in front of a computer monitor.
