Monday through Friday 8:30AM to 5:00pm (40 hours per week) Work Location: Hybrid remote in Santa Clara County, California Responsibilities Provide direct nursing care to clients in community settings Assist in the assessment and evaluation of clients' health needs Develop and implement individualized care plans in collaboration with the healthcare team Monitor and document clients' health status and progress Educate clients and their families on healthcare, wellness, and preventive measures Coordinate with multidisciplinary teams to ensure a holistic approach to care Support clients in accessing mental health and social support services ?
