Vocational Nurse (LVN) to join our Community Health team as part of
the CalAIM ECM Program. In this dynamic role, you will play a
crucial part in delivering essential healthcare services to
underserved populations, ensuring that every individual receives

impact. As an LVN, you will engage in various nursing duties,
contribute to the development and implementation of care plans, and
work closely with clients to promote their overall mental and

Hours:? ? Monday through Friday 8:30AM to 5:00pm (40 hours per
week) Work Location: Hybrid remote in Santa Clara County,
California Responsibilities Provide direct nursing care to clients
in community settings Assist in the assessment and evaluation of
clients' health needs Develop and implement individualized care
plans in collaboration with the healthcare team Monitor and
document clients' health status and progress Educate clients and
their families on healthcare, wellness, and preventive measures
Coordinate with multidisciplinary teams to ensure a holistic
approach to care Support clients in accessing mental health and

valid LVN license in the state of practice Minimum of 1 year
experience in a healthcare or community health setting Strong
understanding of mental health care principles and practices
Excellent communication and interpersonal skills Ability to work

patient-centered approach to care Willingness to travel within the
community as needed Possess a valid California Driver’s License
(Class C minimum), maintain a personal, operable vehicle for daily
business use, and carry current liability insurance that meets
California's minimum legal requirements. All selected candidates
will be required to pass a Motor Vehicle Report (MVR) background
