Job Description:
The Inpatient Case Manager assess, plan, implement, monitor &
evaluate options & services to affect an appropriate,
individualized plan for hospital patient care across the continuum
of care. The Inpt CM will follow assigned patients working w/team
members & Social Services to prioritize & strategize each day’s
activities. Using independent judgment & discretion, the patient
must be assessed & reassessed as an ongoing process to address the
patient’s broad spectrum of needs. The Inpt CM utilizes clinical
knowledge & competence, positive communication skills, problem
solving & conflict resolution techniques, ability to effect change,
strong skills in assessment, organization, & time management. The
CM process requires a focus on critical thinking skills, customer
service skills, setting appropriate goals & measuring outcomes to
effectively ensure optimal patient outcomes w/consideration to
financial & health plan resources. Works under the supervision of
the Accounts Payable Supervisor. Responsible for preparing and
issuing all disbursements with proper authorization in a timely
manner. Ensures the accuracy of the amounts to be paid and the
proper coding. Maintains all vendor accounts. Reviews, matches,
codes and keypunches vendor invoices. Researches problems and works
with vendors and staff in professional and timely manner. Maintains
all vendor accounts by reconciling vendor statements on a monthly
basis. Scans all incoming mail daily. Accurately accrues sales tax
for out of state vendors. Produces weekly check run and associated
reports. Matches checks to invoices – verifies invoice detail to
check detail. Also runs month end closing process. Assists the
Accounts Payable Supervisor and management with other duties as
assigned to meet business needs. Physical Effort: The physical

Long hours sitting, lifting up to 35 to 40 pounds. Skills:
Experience: Minimum of 2 years acute clinical nursing experience
preferred. Case Management experience strongly preferred. BSN/MSN
preferred. Licensure: Current California state RN license.
Certified Case Manager (CCM) or Accredited Case Manager (ACM)
preferred. Skills: Basic knowledge of personal computer and
software for word processing and good keyboard skills required.
Ability to enter and retrieve data from relevant computer systems
required. Knowledge of MCG, MIDAS, Cerner, Medicare rules and
regulations and Conditions of Participation. Knowledge of Joint
Commission, Department of Human Services, Patient Safety, and the
Department of Health organizational standards. Knowledge of acute
care, home care, subacute care, long term care, hospice
interventions, rehabilitation options, other community resources
and requirements, medical necessity guidelines, quality assurance
process and determination of positive outcomes, current standards
and trends in health care, best practices, management tools, and
familiarity with related resources and literature. North Bay Way

Graduate of an accredited nursing school or college. Minimum of 2
years acute clinical nursing experience preferred. Case Management
experience strongly preferred. BSN/MSN preferred Licensure: Current
California state RN license. Certified Case Manager (CCM) or
Accredited Case Manager (ACM) preferred. This is the pay range that
RightSourcing (a part of Magnit) reasonably expects to pay someone
for this position, however, as a supplier your expected pay range
may vary and/or include certain benefits like: Stipends (for
clinical traveler workers only), Medical, Dental, Vision, 401K
[include any compulsory benefits such as commissions, incentive
bonuses, etc. if applicable]. Pay range: (USD)57-(USD)85
