Responsibilities of the Travel Case Manager Nurse: Coordinate and
oversee the patient's continuum of care across various healthcare
settings Conduct comprehensive assessments and develop
individualized care plans Collaborate with physicians, nurses,
social workers, and other care team members to ensure seamless care
transitions Monitor patient progress and modify care plans as
needed to achieve clinical and financial goals Provide education
and support to patients and families regarding diagnoses, treatment

regulatory standards, and documentation requirements Participate in

of the Travel Case Manager Nurse: Active California Registered
Nurse (RN) license Current BLS certification Certified Case Manager
(CCM) or Accredited Case Manager (ACM) Minimum of 2 years acute
clinical nursing experience Some weekended coverage needed (Mostly
