Matrix Providers is hiring a Registered Nurse (RN)Disease Mgmt to join our team of talented professionals who provide health care services to our Military Service Members and their families in Fairfield, CA.

Employment Status: Full Time
Compensation: This is an hourly position, paid bi-weekly.
Schedule: Monday - Friday, 730 AM - 4:30 PM

Requirements: The Registered Nurse (RN) in Disease Mgmt must have and meet the following:

Degree: Associate's Degree in Nursing.
Education: Must be a graduate of a college or university accredited by the National League for Nursing Accrediting Commission (NLNAC), or The Commission on Collegiate Nursing Education (CCNE)
Certification: Certification in disease-management-related areas such as Chronic Care Professional (CCP) by the Health Sciences Institute or Certified Health Coach (CHC) by the National Society of Health Coaches; or a patient education body such as Certified Diabetes Educator (CDE) by National Certification Board for Diabetes Educators or Certified Asthma Educator (AE-C) by National Asthma Educator Certification Board is highly desirable. It is highly recommended candidate have at least one of these certifications.
Experience: Six years of broad-based clinical nursing experience with three years of progressively increasing managed care responsibilities with a focus in Disease Management is highly recommended. Experience with disease management, clinical practice guidelines, performance improvement, and personnel management is strongly preferred. Full-time employment in a nursing field for at least 12 of the last 18 months is mandatory. Possess a thorough clinical knowledge and breadth of experience to be able to contribute to population-level assessment and management, identify appropriate use of CPGs, conduct patient self-care education, and track patient outcomes. Extensive knowledge of DM components: outcomes measurement, collaborative practice, CPGs, patient self-management, population identification processes, and process improvement.
Licensure: Current, full, active, and unrestricted license to practice as a Registered Nurse.

Job Summary:

Coordinate patient care in collaboration with a wide array of healthcare professionals. Facilitate the achievement of optimal outcomes in clinical care, quality, and cost-effectiveness
Ensure compliance with standards of care and practice by all established policies, procedures, and guidelines used in the Medical Treatment Facility (MTF).
Perform physical exams and health histories.
Provide health promotions, counseling, and education.
Administer medications, wound care, and numerous other personalized interventions.
Direct and supervise the care provided by other healthcare professionals.
Accountable for making patient care assignments based on the scope of practice and skill level of assigned personnel.
Recognize adverse signs and symptoms and quickly react in emergencies.
Communicate and collaborate with a diverse group of people to inform the healthcare team of plans/actions for teaching/education to benefit the patient/family and organization.
Make referral appointments and arrange specialty care as appropriate.
Perform nursing services as identified in the official Task Order.
Conduct research in support of improved practice and patient outcomes.
