Minimum Qualifications
Any combination of education and experience that would likely
provide the required knowledge, skills, and abilities as well as
possession of any required licenses or certifications is
qualifying.
*Graduate of an approved Physical Therapy curriculum with either a*
*Bachelor's or Master's Degree.*
*Experience: *None Required
*License/Certification: * PTLA - Physical Therapist Licensed
Applicant by the state of Arizona by the Physical Therapy Board of
Arizona
Essential Responsibilities And Expectations
Under The Supervision Of A Licensed Physical Therapist:
* Evaluate Patients Promptly, And Per Facility Policies And
Expectations, Upon Receiving Physician Referral For Treatment
* Develop Effective Treatment Plans
* Obtain Approval For Services From Referring Physician
* Treat Patients According To Treatment Plan Approved By
Physician
* Communicate Regularly With Supervisor, Other Members Of The
Rehabilitation Team, And Staff Regarding Patient Treatment Plan,
Progress, And Prognosis
* Record Evaluations, Daily Treatment Notes, Weekly Progress Notes,
14-Day Progress Report To The Physician, Recertifications, And
Discharge Summaries Per Company And Facility Policy And
Procedure.
* Complete All Required Documentation From Your Daily
Treatments/Interventions Prior To Leaving The Facility
* Educate Patient Families In Treatment And Any Applicable
Maintenance Programs
* Participate In Discharge Planning
* Secure Necessary Durable Medical Equipment (E.G., Orthoses,
Walkers, Canes Elevated Toilet Seats, And Grab Bars) For Patients
To Facilitate Independent Mobility
* Submit All Billing Logs And Necessary Information On A Timely
Basis
* Report Any Problem Areas/Equipment Within Each Facility To The
Appropriate Supervisor Immediately
* Comply With State Licensing Board Of California, Title 22, CMS,
And California Labor Board Guidelines
* Comply With Company And Facility Rules, Policies And
Procedures
* Comply With All Applicable Safety Rules, Policies, And
Procedures
Additional Responsibilities And Expectations
* Under The Supervision Of A Licensed Physical Therapist, Provide
Necessary Training And Guidance To Student Interns As Assigned,
Including:
* Supervising Students During The Internship
* Providing Clearly Defined, Career-Related (I.E., Non-Clerical)
Responsibilities And Tasks In Accordance With The Students'
Academic Programs
* Informing Student Interns About And Ensuring Their Compliance
With Company, Facility, State, And Federal Guidelines, And
* Completing Any Paperwork Required By The Company Or The Students'
Academic Program
* Assume Other Duties As Assigned

Knowledge, Skills, and Abilities
These are the observable and measurable attributes and skills
required to perform successfully the essential functions of the job
and are generally demonstrated through qualifying experience,
education, or licensure/certification.
* Knowledge of principles, methods, equipment, and theory of the
practice of physical therapy
* Knowledge of pathologies and injuries that result in physical
impairment
* Knowledge of evaluation and treatment methodologies as applied to
routine patient care
* Knowledge of regulatory practice requirements
* Knowledge of legal issues affecting the practice of Physical
Therapy;
* Knowledge and adherence to Code of Ethics and performance
standards as outlined by the APTA
* Knowledge of basic business aspects of rehab therapy services,
such as utilization management, charging practices, and regulatory
compliance;
* Knowledge of available equipment and vendors used in assigned
areas.
* Ability to develop and document clearly and accurately treatment
goals that are realistic, measurable, appropriate, functionally
based, and which include patient/family input
* Ability to provide appropriate care and progress treatment based
on professional standards of practice, and on the needs of the
specific individual, including age, cultural and psychosocial
issues, precautions and medical condition'
