  • Status: Full Time
    • Schedule: Monday - Friday, 730 AM - 4:30 PM

          • 401(k) PlanThe Registered Nurse (RN) must have the following qualifications:
            • Degree: Associates Degree of Nursing.
            • Education: Must be a graduate from a college or university accredited by National League for Nursing Accrediting Commission (NLNAC), or The Commission on Collegiate Nursing Education (CCNE)
            • Certification in disease-management related areas such as Chronic Care Professional (CCP) by Health Sciences Institute or Certified Health Coach (CHC) by National Society of Health Coaches; or a patient education body such as Certified Diabetes Educator (CDE) by National Certification Board for Diabetes Educators or Certified Asthma Educator (AE-C) by National Asthma Educator Certification Board is highly desirable. It is highly recommended candidate to have at least one of these certifications
            • Experience: Six years of broad-based clinical nursing experience with three years of progressively increasing managed care responsibilities with a focus in Disease Management is highly recommended. Experience with disease management, clinical practice guidelines, performance improvement, and personnel management is strongly preferred. Full time employment in a nursing field for at least 12 of the last 18 months is mandatory. Possess a thorough clinical knowledge and breadth of experience to be able to contribute to population-level assessment and management, identify appropriate use of CPGs, conduct patient self-care education, and track patient outcomes. Extensive knowledge of DM components: outcomes measurement, collaborative practice, CPGs, patient self-management, population identification processes, and process improvement.
            • Licensure: Current, full, active, and unrestricted license to practice as a Registered Nurse.Due to contractual obligations with the DoD, Health Care Workers performing under this contract must either be a U.S. citizen, or as defined by the Department of Homeland Security, U.S. Citizenship and Immigration Services, either a noncitizen national of the United States, lawful permanent resident of the United States, or alien authorized to work in the United States.You will love being part of the Matrix family of Providers whose focus since day one has been selflessly giving back to our hard-working and devoted military servicemen and women.
